Registered Nurse – Home Health

Job Summary:

Provides coordinated home nursing care based on physician orders, utilizing the nursing process to deliver quality patient services. Responsible for supervising home health aides and licensed practical nurses, ensuring adherence to agency policies and standards.

Responsibilities:

• Coordinate and deliver in-home nursing care to patients, following physicians’ orders and agency protocols.
• Supervise and evaluate home health aides and licensed practical/vocational nurses to maintain high standards of patient care.
• Document patient visits, assessments, and care plans accurately within the required timeframe.
• Travel to patient homes within a 75-mile radius, accommodating a schedule of 5-8 patients daily.
• Participate in periodic office meetings and collaborate with the Clinical Manager to ensure optimal patient outcomes.
• Maintain current licensure, certifications, and proficiency in EMR systems, including Home Care Home Base (HCHB) and Oasis.

Qualifications:

• Valid RN license in the state of assignment, with no pending license issues.
• Minimum of 1 year recent adult home health or hospice experience.
• Proficient in Oasis documentation and experienced with Home Care Home Base EMR system.
• Must have reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Ability to travel up to 75 miles from the home clinic and accept flexible patient schedules.
• Strong communication skills and the ability to work independently in a community setting.
