Social Worker - South Mountain Restoration Center

THE POSITION

Are you ready to support residents and families through meaningful and compassionate service as a Social Worker 1 with the Department of Human Services? At South Mountain Restoration Center, you will help guide individuals through admissions, daily needs, and important life decisions. This role allows you to make a positive impact by providing support, solving problems, and building trusting relationships. Your dedication will help ensure each resident receives the care, attention, and respect they deserve.

This position provides social services to residents by supporting admissions, counseling needs, treatment planning, and discharge processes. The work focuses on communication, coordination, and ensuring residents receive person centered care throughout their stay.

As a Social Worker 1, you will perform the following duties:

• Resident Support:
  Assist residents and families during admissions, provide guidance, and help apply for benefits
• Daily Interaction:
  Assess resident needs, offer counseling, and advocate for personal preferences
• Team
  
  Collaboration:
  
  Participate in interdisciplinary meetings and develop social service goals and interventions
• Clinical Documentation:
  Write assessments and progress notes following professional and policy standards
• Discharge Planning:
  Coordinate discharge needs with the treatment team and outside agencies
• Program Participation:
  Serve on committees, attend meetings, and support facility projects

• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Telework:
  You will not have the option to telework in this position.
• Salary:

• A bachelor's or master’s degree in social work or social welfare.
• Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree.
