- Provide comprehensive home health nursing care to patients in their residence
- Conduct OASIS assessments and ensure accurate documentation
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Monitor patient progress and adjust care plans accordingly
- Educate patients and families on health management and post-acute care instructions
