Home Health RN: OASIS Expert

Schedule:

• Full-time, Monday through Friday
• On-call duties are probable
• Daily structured patient visit expectations

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Demonstrated proficiency with OASIS required
• Experience with Home Care Home Base is strongly preferred
• Capable of managing a structured, full-time caseload
• Reliable transportation and a valid drivers license

Key Responsibilities:

• Provide skilled nursing care and interventions to patients in their homes
• Conduct comprehensive patient assessments and accurately complete OASIS documentation in a timely manner
• Collaborate with physicians, therapists, and the wider care team to create and implement individualized care plans
• Deliver wound care, medication management, disease monitoring, and patient/family education
• Maintain clear communication regarding patient status and updates with the clinical team
