Physical Therapist

We’re hiring a Physical Therapist to provide personalized treatment in community settings. Your role will include comprehensive evaluations, treatment planning, patient education, and collaboration with interdisciplinary teams.

• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, modifying treatment plans as needed.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.
• Educate patients, families, and caregivers on exercises, safety techniques, and injury prevention.
• Maintain accurate documentation and adhere to all legal, ethical, and professional standards.

• Licensed Physical Therapist – must hold a valid license in the state of Michigan.
• Experience – a minimum of one year in home health care is preferred, but open to new talent.
• Excellent communication skills and ability to engage with patients and families effectively.
• Compassionate caregiver with a desire to promote independence.
• Self‑starter who can work autonomously, manage own schedule, and track patient progress.
• Valid driver’s license and reliable vehicle for travel to patient locations.
