Overview

Sentara Health Plans is hiring an Integrated Nurse Case Manager/RN for Maternal Health/Perinatal in Harrisonburg and surrounding areas. The position is full‑time, permanent (40 hrs), 8am‑5pm EST, M‑F.

Location

Applicants must reside in Harrisonburg, Staunton, Lexington, Rockingham, Stuarts Draft, Luray, VA, and the surrounding areas.

Responsibilities
  • Provide case management services within the scope of licensure.
  • Develop, monitor, evaluate, and revise member’s care plan.
  • Perform telephonic or face‑to‑face clinical assessments for low and high risk pregnant populations.
  • Identify high‑risk complications and coordinate care with the member and the health‑care team.
  • Manage chronic illnesses, comorbidities, and disabilities to ensure cost‑effective utilization of benefits.
  • Assist with implementation of care plans, facilitate authorizations/referrals.
  • Interface with Medical Directors, Physician Advisors, and inter‑disciplinary teams on treatment plans.
  • Present cases at case conferences for multidisciplinary focus.
  • Ensure compliance with regulatory, accrediting, and company policies and procedures.
  • Assist in problem‑solving provider, claims or service issues.
Qualifications
  • Bachelor’s Degree in Nursing preferred.
  • Registered Nurse (RN) license (Compact or Virginia).
  • 3+ years of nursing experience.
  • Experience in case management preferred;
    Managed Care or Health Plan experience preferred.
  • Experience working with low and high risk pregnant populations preferred.
  • Strong knowledge of physical, psychological, socio‑cultural, and cognitive patient needs.
  • Excellent communication skills, both oral and written.
  • Strong problem‑solving and analytical skills.
