Tryfacta is seeking a Long Term Care (LTC) for our client in Harrisonburg, VA 22802. This is a 12 weeks contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Harrisonburg, VA 22802

Duration: 12 weeks

Work Schedule:

2:00 PM to 10:30 PM

Key Responsibilities:

• Provide direct nursing care to residents in a Long Term Care setting.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the healthcare team.
• Maintain accurate patient records and documentation.
• Assist with resident assessments and care planning.
• Collaborate with physicians, nurses, and interdisciplinary care teams to ensure quality resident care.

Requirements:

• Active LPN License required.
• Long Term Care experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and teamwork abilities.
• Ability to work evening shifts and adapt to resident care needs.
