Part One:
Dimension and Description Primary Purpose

To provide quality patient care by assisting the physician, RN, or LPN.

Formal Policy-Setting Responsibilities

No formal responsibility. The policies associated with the job’s purpose and essential responsibilities are set by others.

Routine Decision Making

Determine when to report patient concerns and abnormal vital signs to the RN, determine appropriate supplies necessity, to maintain patient care.

Formal Supervisory Responsibility

No formal supervisory responsibility.

Required Knowledge

Medical terminology and normal patient vital signs.

Required Skills and Ability

Organization, problem-solving, computer skills, customer service, written and verbal communication skills.

Unusual Working Conditions

Risk of blood, body fluid contamination. Risk for injury from aggressive, combative or disoriented patients.

Education and Certification/ Registration Required for the Position

High school education, CNA certification or have completed first semester of nursing school program. BLS required.

Age Specific Competencies

May provide care for infants (birth-2 yrs.), pediatric/child patients between (2 yrs-12 yrs); adolescent patients between 12 to 18 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.

Part Two:
Essential Responsibilities and Tasks

A. Assists in implementation of plan of care for all assigned patients daily. (60% of time)
  • Assists with activities of daily living throughout shift.
  • Assists patients with activity order as prescribed by physician each shift.
  • Obtains vital signs/weights on assigned patients as ordered daily.
  • Reports any abnormal findings to team leader immediately.
  • Assists staff nurses and/or physician on each shift.
  • Ensures patients have fresh water throughout shift, if physician orders allow.
  • Ensures each patient has an identification bracelet upon admission to the unit and when bands are reviewed.
  • Updates white boards each shift.
  • Utilizing the AIDET model, conducts an appropriate introduction, discusses with patient ADL's and plan for the shift and compliments at least one member of the patient care team each shift. Purpose directly relates to patient satisfaction scores and performance evaluation
  • Bedside report to occur every shift between oncoming and off-going staff
  • Attendance of staff meetings:
    Part of performance review. 3 = 6 or more meetings, 2 = 4-5 meetings, 1 = less than 4
B. Maintains a safe and comfortable unit. (25% of time)
  • Keeps patient rooms and hallway free of clutter daily.
  • Ensures alarms on any assigned areas are audible to alert staff, each shift.
  • Removes malfunctioning equipment from the direct patient care area immediately upon discovering a problem.
  • Completes an electronic requisition for repair and provides a copy to manager before the end of shift.
  • Utilizes correct patient identification by the hospital approved, two methods, for positive patient identification.
  • Acts rapidly and calmly during emergencies.
C. Demonstrates accountability to the nursing process through documentation. (5% of time)
  • Utilizes only approved abbreviations in patient care documentation each shift.
  • Documentation follows hospital and departmental policies for frequency and content.
D. Develops and promotes performance improvement initiatives. (10% of time)
  • Plans, develops and evaluates self-objectives, annually.
  • Participates in planning ways to solve problems and make improvements in the medical surgical unit, quarterly, and as needed.
  • Participates in implementing planned changes and activities to improve the medical surgical unit quarterly, and as needed.
