    RN degree required, BSN preferred. Current RN licensure in Missouri or compact licensure required. BLS, ACLS, PALS and NIH Stroke Certification required within 90 days of hire.
Essential Responsibilities and Tasks

A. Assessment Phase (20% of time)
  • Ensure all patients are placed on the Med Surg Unit within thirty minutes of notification of admission/transfer if a bed is available.
  • Complete a patient admission assessment within four (4) hours of arrival to the unit, including pertinent history, assessment of body systems, medication history and screenings.
  • Assess data relative to age‑specific needs for every patient within four (4) hours of arrival.
  • Assess patient/family learning needs within four (4) hours of arrival.
  • Complete appropriate pain assessment of each admission, during each shift and as needed.
  • B. Planning Phase (15% of time)
  • Complete an individualized care plan based on diagnosis and assessment with each admission, during each shift and as needed. Approve care plan for new patients assigned to LPNs.
  • Collaborate with interdisciplinary team to facilitate plan of care after reviewing provider’s admission orders and required screens.
  • Interpret significance of abnormal findings during assessment.
  • Use AIDET model to conduct introductions, review plan of care and set attainable patient goals.
  • Update white boards each shift to include personalized goals and pain control.
  • Conduct bedside report with incoming and outgoing staff every shift.
  • C. Implementation Phase (20% of time)
  • Implement patient care throughout each shift.
  • Document and report significant abnormal findings to provider immediately.
  • Know location, care and operation of Medical Surgical equipment.
  • Act rapidly, effectively and manage self and others during emergencies.
  • Administer IVs and medications within one hour of scheduled time according to the five rights.
  • Ensure all provider orders are correctly recorded on EMAR and acknowledged in the computer.
  • Follow fall protocol for each patient.
  • Waste controlled substances properly and conduct narcotic counts per policy.
  • Attach patient identification bands (name, fall risk, allergy) immediately after arrival and remove when appropriate.
  • Assign patients to staff each shift.
  • Facilitate interdepartmental communication regarding patient care.
  • Assist with discharge process for all patients.
  • Address patient/Provider concerns and escalate to manager if needed.
  • Round on patients as needed.
  • D. Patient and Family Education (5% of time)
  • Provide education based on assessments each shift and as needed.
  • Use terminology appropriate to patient/family knowledge and age.
  • Re‑assess learning needs and abilities each shift and as needed.
  • Document education provided: subject, method, who was taught, comprehension.
  • Provide and document follow‑up instructions at discharge.
  • E. Documentation (15% of time)
  • Document interventions clearly with time, methods and patient response.
  • Use only approved abbreviations in documentation each shift and as needed.
  • Follow hospital and departmental documentation policies for frequency and content.
  • F. Unit Maintenance (15% of time)
  • Check code blue carts each shift and verify intact tag.
  • Ensure alarms in assigned areas are audible each shift.
  • Remove malfunctioning equipment immediately; submit repair requisition before shift end.
  • Complete job duty list for the unit each shift.
  • Use correct two‑method patient identification for positive .
  • Use barcode scanning for medication administration via EMAR 97% of the time.
  • Complete patient assignment record each shift.
  • Assess staffing needs based on plan each shift.
  • Provide thorough handoff communication to next shift.
  • G. Unit/Hospital Initiatives (10% of time)
  • Plan, develop and evaluate self‑objectives annually.
  • Participate in problem‑solving and improvements quarterly and as needed.
  • Implement planned changes and activities to improve unit quarterly and as needed.
  • Evaluate changes and activities quarterly and as needed.
  • Attend unit staff meetings; attendance tracked by performance ratings.
