Purpose:
To provide quality patient care by assisting the providers, RN, or LPN. Performs other duties as assigned.
Formal Policy-Setting Responsibilities:
No formal responsibility. The policies associated with the job's purpose and essential responsibilities are set by others.
Routine Decision Making:
Determine when to report patient concerns and abnormal vital signs to the nurse, determine appropriate supplies necessary to maintain patient care.
Formal Supervisory Responsibility:
No formal supervisory responsibility.

Working Conditions:
Risk of blood, body fluid contamination. Risk of injury related to poor body mechanics. Risk for injury from aggressive, combative or disoriented patients. Potential exposure to hazardous drugs and chemicals.
Age Specific
Competencies:
May provide care for infants (birth-2 yrs.), pediatric/child patients between (2 yrs-12 yrs.); adolescent patients between 12 to 18 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.
Assists in implementation of plan of care for all assigned patients daily. (60% of time)
- Assists with activities of daily living throughout shift.
- Assists patients with activity order as prescribed by provider each shift.
- Obtains vital signs/weights on assigned patients as ordered daily.
- Reports any abnormal findings to team leader immediately.
- Assists staff nurses and/or providers on each shift.
- Ensures patients have fresh water throughout shift, if provider orders allow.
- Ensures each patient has an identification bracelet upon admission to the unit and upon bands being removed.
- Updates white boards each shift.
- Utilizing the 'AIDET' (Acknowledge, Introduction, Duration, Explain, Thank) model, conducts an appropriate introduction, reviews the plan of care, mutually sets attainable patient goals.
- Bedside report and patient rounding to occur every shift between oncoming and off-going staff.
