Job Details

Responsibilities:

Provide comprehensive outpatient primary care to adult patients
Deliver preventive care, wellness visits, and chronic disease management
Collaborate with a multidisciplinary care team to ensure continuity of care
Utilize EMR systems for efficient, high-quality documentation
Participate in quality initiatives and patient-centered care models

Qualifications:

MD or DO
Board Certified or Board Eligible in Family Medicine or Internal Medicine
Eligible for or licensed in Connecticut
Strong commitment to outpatient primary care and patient experience
