The Cancer Community Outreach Manager is responsible for managing system-wide community programs such as the Connecticut Early Detection and Prevention Program (CEDPP) and WISEWOMAN, Hartford Hospital's Prostate and Colorectal Screening Program and Mobile Mammography program. The Community Manager plans and develops outreach strategies; including defining the goals and timelines for the programs managed; collaborating with the Department of Public and other statewide organizations and providers.
Responsibilities include but, are not limited to the following:
* Cultivate and sustain partnerships with diverse stakeholder groups (e.g. community organizations, schools, health organizations, and churches) to increase program and hospital awareness
* Oversees Hartford Hospital's Mobile Mammography Program in collaboration with the Department of Radiology.
* Assist in the development and administration of assigned program budget; monitor and approve expenditures; recommend budget adjustments as necessary.
* Convene and participate in meetings and committees to implement plans and identify additional collaborative opportunities that promote programs and align with Cancer Committee Standards
* Apply for grants to ensure funding for Connecticut Early Detection and Prevention Program, Prostate and Colorectal Screening Program.
* Develop work plans, and implement CEDPP throughout Hartford Health Care System hospitals and subcontracted providers.
* Oversees the completion of monthly and quarterly reports and provides the Department of Public Health quarterly reports.
* Maintains outreach database for tracking and trending purposes to assure programs are meeting the needs of the community.
* Implements and monitors outreach goals and objectives that align with the strategic direction of the organization by analyzing needs and developing recommendations.
Education:
Bachelor's degree in Communications, Social Work, Human Services or related field required. Master's degree, Master's in Public Health (MPH) preferred
Experience:
Minimum 2 to 5 years community outreach experience
Professional proficiency in both verbal and written English and Spanish strongly preferred
