About the Role

We are seeking passionate, skilled clinicians to join our Adult Services team. This position offers two unique pathways—both combiningoutpatient clinical work with impactful community-based services.

Option 1:
Adult Outpatient Clinic + Acute Community Support Team (ACST)

50% providing individual, group, and case management services inour Adult Outpatient Clinic

50% delivering higher-level, community-based care through ACST,supporting adults with Severe and Persistent Mental Illness

Engage clients in homes, shelters, and community settings

Support access to treatment, stabilize symptoms, coordinate care,and foster improved quality of life

Model based on SAMHSA's ACT framework

Option 2:
Adult Outpatient Clinic + Contracted Community Program

50% providing outpatient clinical services

50% working off-site with young women (ages 16–24) who have experienced significant trauma

Support clients involved with state systems, young parents, andthose with limited employment history

Build trust through community-based engagement (in-home, in-shelter)

Collaborate closely with program staff to coordinate care and strengthen behavioral health outcomes

Key Responsibilities

Conduct comprehensive clinical assessments and engagement

Determine DSM-V diagnoses in collaboration with supervisory ormedical staff

Develop, implement, and adjust treatment plans

Provide case management and discharge planning, includingcommunity linkages

Complete all required clinical documentation in accordance with agency, HIPAA, and funding standards

Meet productivity expectations

Participate in peer consultation and quality assurance reviews

Collaborate with clinical and administrative teams to addressclient and program needs

Identify service gaps and contribute to program improvements

Maintain competency in evidence-based models used by the agency

Engage in professional development planning with supervisor

Contribute to agency committees, task forces, and community partnerships

Experience and Skills Education & Qualifications

Master's degree in social work, psychology, or related counseling field

Active Connecticut license required (LCSW, LPC,LMFT, Psychologist)

Minimum 2+ years post?graduate outpatientexperience (program?dependent)

Bilingual/bicultural candidates strongly encouraged

Demonstrated cultural competency and experience with diverse populations

Strong diagnostic, assessment, and therapeutic skills acrossindividual, family, and group modalities

Knowledge of behavioral health and child welfare systems, and community resources

Excellent communication and organizational skills

Ability to work independently while seeking consultation as needed

Proficiency with basic computer and documentation systems
