Position Summary:

Home Health Aide / CNA Job Description

A Home Health Aide is a non‑licensed member of the home care team who assists the client with daily living tasks as outlined in a written care plan. The care plan is established by the Administrator or CSS and kept at the client’s home. Home Health Aides are supervised by the Administrator agency and receive scheduling direction from the agency staffing coordinator.

Qualifications

A Home Health Aide must have a minimum of one year of personal care experience, verifiable through three work references, and must complete and pass the agency’s employment and competency process.

Must be at least 18 years of age.

A Home Health Aide must have training as a Home Health Aide and complete a competency evaluation.

Must be physically able to perform the duties of the position, including lifting unspecified weights, bending and stooping to prepare equipment, transferring, turning and positioning clients, and assisting ambulation.

Must exhibit mature, responsible behavior and understand patient confidentiality.

Must be able to read, write and speak English.

Must be able to follow direction and accurately report to the Administrator any changes in the client’s condition.

Must have reliable transportation to and from assignments; may be approved to transport clients or run errands in a vehicle.

Certified Nursing Aide (preferred)

Essential Functions

The HHA shall be assigned to a client by an RN. Written instructions for client care shall be prepared and updated as needed by an RN.

The HHA may provide services that are ordered by the physician in the plan of care, that the HHA is permitted to perform under state law, and consistent with the HHA’s training.

HHA services provided to clients may include, but are not limited to, the following duties:

Hands‑on personal care including grooming and hygiene (skin, mouth, hair care, shaving, dressing, bathing).
Performance of simple procedures as an extension of therapy or nursing services.
Assisting clients in and out of bed, ambulation, and exercises.
Nutrition, hydration and assisting clients with eating.
Assisting clients with toileting and elimination.
Performing household services essential to health care at home to facilitate client self‑care and prevent or postpone institutionalization.
Assisting clients with medication reminders for medications normally self‑administered, per the Medication Policy.
Supervision of self‑administered medication in the home limited to obtaining medication containers, ensuring correct prescriptions, reminding the client, and observing self‑administration.

In addition to the requirements outlined in the Orientation Policy, the HHA must demonstrate competency in:

Bathing, shaving, grooming and dressing.
Hair care, skin, nail and mouth care.
Safe ambulation and transfer techniques.
Nutrition, hydration, meal preparation and feeding.
Toileting and elimination.
Recording vital signs (temperature, pulse, respiration).
Medication reminder techniques for self‑administered medications.
Safe techniques in personal care tasks.
Normal range‑of‑motion and positioning.
Prevention of skin breakdown, contractures and falls.
Reporting changes to the Support Services Supervisor or Clinical Manager.
Additional training identified by the agency as necessary.

The HHA may NOT provide:

Change sterile dressings.
Irrigate body cavities, perform a gastric irrigation or enteral feeding.
Catheterize a client.
Administer medication.
Apply heat by any method.
Care for a tracheostomy tube.
Provide any health service not included in the plan of care.
