POSITION SCOPE:

Direct and oversee central intake and admissions operations across all Chrysalis Center programs, ensuring appropriate, timely, and coordinated access to services agency-wide. Supervise intake and outreach staff, establish and monitor admissions policies and procedures, and serve as the primary point of accountability for outreach, intake, aftercare coordination, and related reporting across the agency's full continuum of care.

ESSENTIAL RESPONSIBILITIES Direct agency-wide intake and admissions operations, ensuring timely, consistent, and appropriate access to Chrysalis Center services.

Develop, implement, and maintain intake, admission, eligibility, and aftercare policies and procedures in compliance with agency and funder requirements.

Oversee completion of all interagency referrals, external CAN referrals, including assessment, referrals, and follow-up communication and documentation.

Supervise and coordinate intake/aftercare staff, including hiring, training, scheduling, performance management, and coverage.

Oversee eligibility, assessments, applications, admissions, referrals, and follow-up, ensuring appropriate placement within Chrysalis Center or community services.

Manage agency-wide outreach efforts and maintain relationships with referral sources, hospitals, treatment facilities, managed care organizations, and community partners.

Oversee aftercare coordination, including outreach, assessment, referrals, follow-up, documentation, training, and service continuity.

Ensure accurate and consistent documentation, including intake paperwork, client charts, referral records, and discharge records.

Ensure reception and program staff have current, accurate referral

Monitor and report agency-wide intake, outreach, admissions, aftercare, and client participation statistics, including required funder reports.

Provide leadership and representation for intake and admissions, participating in agency meetings, community activities, trainings, and evening/weekend admission functions.

Maintain accountability for agency-wide intake and admissions outcomes, including quality assurance, policy compliance, safety, equipment, and continuous improvement.

Adheres to all Chrysalis Center, Inc.’s policies, procedures and standards at all times.

Performs other duties as assigned

Master's degree in social work, counseling, public health, or related field, plus a minimum of five years of experience in behavioral health services, with at least two years in a supervisory or management capacity.

Fluency in Spanish is required

Demonstrated ability to work in partnership with staff, clients and the community.

Demonstrated ability to oversee assessments of clients experiencing mental illness, HIV+/AIDS, chemical addictions and homelessness.

Strong knowledge of assessment and crisis intervention methods, levels of care criteria, community resources and service planning across multiple program types.

Excellent interpersonal, organizational, oral and written communication skills; fluency in Spanish is required.

Prior leadership and staff supervision experience required.

Must be able to lift over 30 pounds and carry objects.

Proficiency managing Microsoft and other computer applications used for tracking data and reporting outcomes.

Possess and maintains a valid drivers’ license, reliable vehicle, current registration and insurance.

Demonstrates ability to adhere to strict Confidentiality and HIPAA Guidelines.
