Serves as a member of the multidisciplinary care team with a focus on individualized care of cardiac surgery patients. Provides ongoing patient/family education and supports the coordination of care, as directed by the cardiac surgeon and cardiac surgery provider team.
- Provide pre‑op education to cardiac surgery patients/families primarily in the ambulatory setting with a focus on providing excellent patient/family experience.
- Collaborate with other health care providers (Vascular Surgery, Cardiothoracic Surgery, Heart Failure, Interventional Cardiology, Hospitalists, Nursing, Social Workers, Case Coordinators, Pharmacy, Rehabilitation Medicine, Palliative Care, etc.) to decrease in‑patient length of stay and readmissions of cardiac surgery patients by facilitating a smooth transfer of care to and from the community.
- Identify specific patient and/or family psychosocial, interpersonal and clinical care needs during the preop, intraop and post op cardiac surgery period. Develop plan of care to address these needs and potential barriers.
- Facilitate post‑op ambulatory care of cardiac surgery patients and provide post‑op education as needed.
- Demonstrates the ability to assess, plan and implement plan of care with regard to educational and psychosocial needs of the cardiac surgery patients and their families.
- Demonstrates excellent customer service skills including superior telephonic and written communication skills.
- Functions as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
- Participates in community education (educational outreach programs, workshops, seminars, support services).
- Outpatient nurse navigator will also collaborate with inpatient navigator team to provide coverage in hospital as needed.
- BSN
- 5 years nursing experience
- Licensed as a professional RN in the state of Connecticut
