Primary Care Nurse Practitioner or Physician Assistant

We are hiring a Primary Care Nurse Practitioner / Physician Assistant for a full‑time outpatient opportunity in Hartford, CT. This role is ideal for clinicians who value continuity of care, patient education, and a community‑focused practice environment.

Salary: $110,000 – $175,000 per year. Loan repayment eligible. 4 % 401(k) match.

• Provide comprehensive primary care visits, assessments, and ongoing management for adult patients
• Diagnose and treat acute and chronic medical conditions
• Order and interpret diagnostic tests and labs
• Prescribe medications and manage treatment plans as appropriate
• Educate patients on health maintenance, prevention, and chronic disease management
• Collaborate with physicians, nursing staff, and the broader care team to ensure continuity of care
• Maintain accurate, timely documentation within the EMR

• Active or eligible Connecticut licensure as a Nurse Practitioner or Physician Assistant
• Prior experience in Primary Care, Family Medicine, Internal Medicine, or a related outpatient setting preferred
• Strong clinical judgment, communication, and patient education skills
• Comfortable managing a broad adult primary care panel
