Location:
80 Seymour Street (10008). Shift: Monday–Friday, 3 8‑hour shifts 7 am–3:30 pm.
Our nurses work in collaborative environments with strong interdisciplinary teams and a patient‑centered care model. The registered nurse, under the direction of the Nurse Manager, provides evidence‑based care in accordance with the State Nurse Practice Act, hospital policies, and medical staff directives.
Responsibilities- Complete initial assessment for patients referred to the outpatient cardiac rehabilitation program.
- Identify indications and contraindications for program participation, set specific monitoring needs, and modify risk factor goals for each patient.
- Conduct patient education and counseling, document treatment plans and measurable goals tailored to each patient’s needs.
- Monitor and document patients’ response to exercise therapy before, during, and after sessions, including vital signs, telemetry, oxygen saturation, weight, blood sugar, and support device metrics.
- Report significant acute conditions (e.g., dysrhythmias, blood pressure changes, angina, weight gain) to the supervising physician immediately and report ongoing abnormal trends via event report.
- Initiate ACLS measures or emergency care in accordance with hospital policy.
- Coordinate tasks related to patient recruitment, insurance pre‑certification, registration, and records management (Scott Care, Epic).
- Maintain current knowledge of cardiac rehabilitation through journals, in‑services, mock codes, conferences, and staff meetings.
- Current Connecticut State RN license.
- Current BLS/ACLS certification.
- BSN required; MSN preferred.
- Previous cardiac rehab experience with knowledge of exercise therapy preferred.
- Previous experience in a cardiovascular clinical area (e.g., Coronary Care, Cardiac Rehab, Congestive Heart Failure) for at least two years.
- Meets the skills criteria for Cardiac Rehab Nurse Competencies.
- Capable of meeting the physical and emotional demands of the position.
- Desire to work within a comprehensive healthcare team.
