POSITION SUMMARYThe LPN is responsible for delivery of clinical care and works closely with medical providers, medical assistants and support staff to ensure the effective and efficient delivery of primary health care services to all patients.
Eligible for a $100 scrub uniform allowance.
Rate $30
EDUCATION AND EXPERIENCE/QUALIFICATIONSGraduation from an accredited licensed practical nurse program required. Associates in a related field preferred. Must have 2 years of clinical experience providing nursing services to patients from diverse economic and cultural backgrounds, as well as exceptional communication, interpersonal and organizational skills are required. Outpatient primary care settings experience preferred. Bi-lingual English/Spanish a plus.

Works under the supervision of a Registered Nurse, and performs orders given by the medical provider.
Contributes to the creation of a compassionate and caring environment for patients and families through display of kindness and active listening.
Provides direct care to patients, carrying out orders given by the medical provider, including obtaining patient medical histories, obtaining vital signs, performing blood draws, preparing patients for and performing diagnostic testing, assessing and providing wound care, dressings, administering injections and assisting medical provider in the examination room.
Demonstrates effective teamwork by interacting in a positive manner with colleagues, visitors and patients, caregivers and families.
Contributes to a collaborative work environment by initiating open communication, a positive intent and offering assistance to support patients, families and care givers.
Contributes to a safe and secure work environment for patients, visitors and colleagues by following established procedures and protocols.

Provides chronic care disease support to the patients by phone, in person and in all aspects of the care, treatment and services provided and in accordance with the clinic’s integrated model of care. Develops patient focused self-management goals and care plans with the patient.
Ensures gaps in care for screenings, chronic care tests, (such as HgbA1c for Diabetics), health literacy assessments, tobacco screenings, BMI, etc. are completed in timelines established by best practice.
Attends and provides input for patients in multi-disciplinary team meetings weekly, daily multi-disciplinary huddles.
