Job Overview

Signify Health, part of CVS Health®, is hiring a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. Applicants must reside in Massachusetts and hold active, unrestricted licenses in the coverage area.

• Travel up to 100% of the time within a defined coverage area, including overnight stays, car rentals or flights as required.
• Conduct 45‑minute in‑home visits, reviewing medication, medical history, performing physical evaluation and any ordered diagnostic tests (e.g., diabetic eye exam, spirometry).
• Use iPad and intuitive clinical workflow; no prescribing, lab ordering or treatment alterations.
• Provide diagnostic and preventive services such as spirometry, DEE, etc.
• Perform virtual visits as needed.
• Work with Signify Health’s clinician colleagues and support national outreach goals.
• Receive incentive pay per completion and additional per‑visit incentives.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses as required.
• Coverage within all counties of Massachusetts; flexibility to cover additional business needs.

• Multi‑state licensure coverage for the listed MA counties.
• Experience with in‑home clinical evaluations.

Full‑time, 40 hours per week. Anticipated work hours 40 per week. Pay range: $95,738.00 – $ (base hourly rate or annual salary). Sign‑on bonus eligible.
