About the Role:As a Home Health Registered Nurse, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, educating patients and families and supporting recovery and independence.
Whether you’re experienced in home health or transitioning from hospital, acute care or skilled nursing, this role offers the chance to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationships.

- Graduate of an approved nursing education program, BSN preferred.
- Licensure as a Registered Nurse in the State of Connecticut.
- One (1) year experience as a professional acute care nurse.
- Home care experience preferred but not required.
- Must have current Driver’s license and reliable transportation.
