Job Summary:
Provides diabetes education to patients, families, hospital staff and in the community using current intervention techniques to restore, maintain and promote optimal health in persons with diabetes. Reports to Manager.
Job Responsibilities:
- Acts as an educational resource to hospital staff, patients and family members by collaboratively planning and implementing patient care to assure promotion, restoration and maintenance of health through Diabetes Self-management skills.
- Conducts in services through formal and informal education to assure hospital staff, patients and family members develop knowledge and skills for Diabetes Self-management.
- Screens and teaches diabetes management skills by promoting diabetes education in the outside community to assure newly diagnosed persons are restored to optimal health.
- Develops, assesses, revises and evaluates teaching programs and services by collaborating with Manager to assure individual and group needs are being met according to appropriate standards.
- Maintains appropriate documentation by demonstrated adherence to department and regulatory standards to assure patient safety and compliance.
- Works efficiently as a team member both within and across clinical settings to provide quality service to hospital units through communication, cooperation and collaboration.
- Demonstrates competence, including knowledge and behaviors, to meet age specific patients needs of toddler, school age, adolescent, adults and geriatric population.
Qualifications:
Current Connecticut RN License Bachelors of Science Degree in Nursing Masters of Science preferred Certified Diabetes Educator credential applied for within two years of employment, and obtained within 3 years of employment. Minimum of two years recent clinical experience in medical-surgical area. Strong communication skills required Bilingual in Spanish helpful
