Seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a full-time Post-Acute Care opportunity in the Hartford, Connecticut area.

• Productivity-based compensation (Fee for Service)
• Expected compensation range: $158,000–$188,000 annually with no cap on productivity income potential

• Full-time post-acute care position
• Round in skilled nursing and long-term care facilities
• Conduct patient assessments, develop treatment plans, and manage ongoing care
• Collaborate with physicians, nursing staff, and interdisciplinary care teams
• High level of autonomy with strong operational support
• Connecticut NP or PA license required (or ability to obtain)
• DEA required
• Experience in post-acute, primary care, hospital medicine, emergency medicine, or geriatrics preferred
