Complete Care is seeking an experienced and relationship-driven Registered Nurse (RN) to join our team as a PRN Nurse Liaison supporting our Connecticut region. This role focuses on external case management, hospital partnerships, and referral development
, stepping in to provide coverage during vacancies, vacations, and leaves of absence
.
This is an ideal opportunity for an RN who enjoys working outside of a traditional facility setting, building relationships, and facilitating smooth patient transitions into post-acute care.
Key Responsibilities- Serve as a clinical liaison between hospitals, case managers, and Complete Care facilities
- Build and maintain strong relationships with hospital discharge planners, case managers, and referral sources
- Review clinical referrals and assess appropriateness for admission to skilled nursing facilities
- Coordinate and facilitate patient transitions from acute care settings to Complete Care centers
- Provide clinical insight and support to ensure appropriate level of care and successful placements
- Maintain consistent communication with internal admissions and clinical teams
- Represent Complete Care in the field, promoting services and strengthening referral pipelines
- Provide coverage across the region as needed to support business continuity
- Active Registered Nurse (RN) license (Connecticut required; multi-state licensure a plus)
- Prior experience in case management, nurse liaison, admissions, or discharge planning strongly preferred
- Background in long-term care, skilled nursing, or acute care required
- Strong understanding of post-acute care criteria and insurance/Medicare guidelines
- Excellent interpersonal and relationship-building skills
- Highly organized with the ability to manage multiple referrals and priorities
- Willingness to travel throughout the Connecticut region
- Flexible PRN schedule
