Registered Nurse (RN) & Licensed Practical Nurse (LPN) – Home Care Company

Affirmed Home Care is a premier provider of compassionate in‑home care serving clients in Hartford, Connecticut and the surrounding areas. We are expanding our team and are looking for dedicated RNs and LPNs to provide one‑on‑one care in the comfort of our clients’ homes.

• $38–$45 per hour for LPNs, based on experience
• $55–$65 per hour for RNs, based on experience

• Conduct thorough client assessments and implement individualized care plans.
• Administer medications, treatments, and nursing procedures in accordance with physician orders.
• Monitor, evaluate, and document client conditions, communicating updates promptly to care teams.
• Provide advanced nursing interventions, including wound care, IV therapy, and catheter management (RNs).
• Educate clients and families about health maintenance, medication compliance, and home safety.
• Maintain accurate, detailed documentation following regulatory and agency standards.
• Ensure compliance with all state, federal, and company healthcare protocols.
• Provide compassionate, patient‑centered care that reflects Affirmed Home Care’s values of dignity and excellence.

• Active RN or LPN license in the State of Connecticut (required).
• Minimum 1–2 years of nursing experience (home care, geriatric, or related field preferred).
• Strong assessment and clinical decision‑making skills.
• Excellent communication, time management, and interpersonal abilities.
• CPR/BLS certification (required).
• Reliable transportation and a valid driver’s license.
• Experience with Electronic Health Records (EHR) preferred.
