Job Description

MDS Care Plan Coordinator leads the MDS department, coordinates discharge planning, and assesses short‑term residents. The role focuses on maximizing reimbursement while ensuring Medicare compliance and accurate completion of the MDS. Responsibilities include gathering information, assessing needs, establishing goals, providing interventions, and integrating these into an organized, concise care plan. The Coordinator coordinates the comprehensive assessment by the interdisciplinary team, incorporates recommendations, and ensures all nursing personnel are aware of and follow the care plans with daily nursing services.

This includes reviewing nurses’ notes, monitoring residents to confirm plan adherence, and revising plans as necessary. The Coordinator plans, schedules, and conducts weekly care plan meetings for all residents, in accordance with OBRA and state requirements. The role also involves completing the MDS with utmost accuracy to secure the highest level of reimbursement and complying with current CMS Mega Rule guidelines.

Point‑Click Care experience is a plus.

Qualifications
  • Current RN or LPN license, in good standing.
  • Full compliance with all applicable federal and state licensure requirements.
  • Attention to detail, strong follow‑through skills, and ability to prioritize multiple tasks.
  • Ability to instruct and lead others.
  • Knowledge of general, rehabilitative, and restorative nursing and medical practices, procedures, laws, regulations, and guidelines governing long‑term care.
