Job Overview
The Care Manager, RN conducts initial in-home assessments, orientations for new patients and caregivers, and provides ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care. The Care Manager collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate seamless initiation and delivery of services.
Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors quality and effectiveness of care, identifies opportunities to enhance health outcomes, safety, and regulatory compliance, and proactively communicates clinical concerns, changes in condition, and potential barriers to care with appropriate team members to ensure timely intervention and optimal outcomes.
- Conduct initial in-person clinical assessments and coordinate service activation, ensuring required forms and procedures are followed.
- Develop, implement, manage, and revise the patient’s plan of care, documenting changes as needed.
- Coordinate the care of the client by communicating and documenting with physicians, other disciplines, and community resources.
- Provide ongoing coaching to promote best practices, encourage caregiver education, and connect families to appropriate resources.
- Review caregiver notes daily and address changes in the consumer’s condition or environment appropriately.
- Maintain continuous compliance with all regulatory standards as well as company policies and procedures.
- Perform other job-related duties as may be required or directed by the supervisor.
- Diploma from an accredited nursing program.
New graduates are welcome.
Preferred Experience- Home care case management
- Care coordination experience
- Valid and active Registered Nurse (RN) license in the State of Connecticut.
- Ability to articulate cases through effective case summaries.
- Ability and flexibility to travel (usually by car) within an assigned geographic area; valid driver’s license and car insurance required.
- Proficiency in common software and web-based applications (e.g., case management systems, MS Office) and ability to teach others.
- Passion for providing a high level of customer service, including empathy, active listening, and patience.

$80,000—$90,000 USD per year.
