$5,000 sign‑on bonus paid over two years.

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.

• Follows care plan developed by RN.
• Reports client changes in condition to RN.
• Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, dressing and undressing.
• Assists clients with household tasks such as planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
• Measures and records client oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
• Performs range of motion and other simple procedures as an extension of therapy services as assigned.
• Assists clients in self‑administration of medication assigned by an RN. Aides shall not administer medication of any kind.

• High school diploma or GED required.
• Active CT CNA certification in good standing.
• Completion of a federally compliant training program, and successful completion of a competency evaluation program.
• Documentation of one year’s employment as an aide in an institution, home‑care, or hospice setting.
• Must have a current driver’s license and reliable transportation.
