About This Role
The Transitional Care Nurse Coordinator for the Congestive Heart Failure Clinic plays a crucial role in managing heart failure patients, focusing on patient-centered care and collaboration within interdisciplinary teams. This position is ideal for experienced nurses who excel in critical thinking and patient advocacy.

- Department/Unit: Congestive Heart Failure Clinic
- Schedule: Full Time, Day shift
- Independently obtain and perform assessments while triaging and guiding a patient’s clinical course.
- Present patient history and clinical presentation to a Provider.
- Establish, implement, and evaluate an individualized plan of care with timely clinical advice and interventions.
- Perform extensive review of medications and eliminate inconsistencies across transitions in care.
- Communicate effectively with Providers to ensure medication safety and effective reconciliation.
- Facilitate coordination of care and collaborate with Advanced Practice Providers and Physicians.
- Perform IV and medication therapy, assessment, and evaluation of the patient.
- Participate in program growth, referral base, outreach, and protocol establishment.
- Lead heart failure support groups and educational sessions.
- Coordinate the Heart Failure patient experience throughout the Hartford Healthcare continuum.
Education:
- Bachelor of Science in Nursing required
- Minimum of 3 years’ acute care experience preferred
- Minimum of 2-5 years’ heart failure experience preferred
Certifications:
- Current Connecticut Registered Nurse License
- Basic Life Support Certification by the American Heart Association
- American Association of Heart Failure Nurses (AAHFN) Certification (must obtain within 6 months of hire)
