TRINITY HEALTH OF NEW ENGLAND AT HOME is seeking a Home Care RN to deliver primary nursing care in patients' homes under physician orders, with initial assessments and care planning in collaboration with the Population Health Case Manager.
You will supervise aides and LPNs as needed, coordinate with the care team, and follow policy. The role includes a $10,000 sign-on bonus and day-one benefits.
Schedule:
Full-Time, Mon-Fri 8am-5pm with weekend rotation and two holidays.
