Summary:
The Licensed Practical Nurse (LPN) contributes to whole person healthcare by providing evidenced-based nursing skills to care for patients in both the Withdrawal Management and Intensive units.
Essential duties & responsibilities:
- Timely administer medications to patients and monitor patients for reactions or side effects
- Maintain accurate, detailed reports and records
- Record patients' medical information and vital signs.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Receive and transcribe written orders from the medical and psychiatric provider
- Medication Reconciliation process
- Facilitate effective crisis intervention
- Assist the RN in consulting and coordinating with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referrals to other levels of care when appropriate
- Assist the RN in modification of the patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes
- Direct or supervise residential technicians while on shift
- Provide medical re-education to patients as needed
- Performs related duties as required.
* All agency staff are required to attend all mandatory department/agency meetings and trainings*
Schedule:
1st Shift: 7:00 AM - 7:30 PM
Week 1:
Monday, Tuesday, Saturday
Week 2:
Sunday, Wednesday, Thursday

Experience:
- Current Connecticut LPN license
- Basic Life Support (BLS)/ CPR certification
- EPIC experience preferred, but not required
