Location:
Hartford, CT 06101.
Position Type:
Full Time.
Salary Range: $115,000 – $200,000 per year (base + commission/month). Travel:
Road Warrior.
Schedule:
Monday – Friday. No nights, weekends, or holidays.

- Ongoing education and clinical support, including physician-level consult access
- Autonomy with support to deliver independent care with access to physician-level consults.
- Deliver person-centered care using evidence-based practices.
- Collaborate with interdisciplinary facility teams.
- Conduct assessments, develop treatment plans, and manage follow-up care.
- Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs).
- Educate patients, families, and staff.
- Complete timely documentation (within 24 hours).
- Compliance with facility, state and federal regulations/protocols.
- Active license as a Nurse Practitioner (NP).
- Minimum 2 years of post-certification experience.
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred.
- Proficient with EHRs and clinical technology.
- Strong communicator with a collaborative mindset.
