About This Role

The Registered Nurse Pre-Admission Testing (RN) at Hartford Health Care provides holistic, quality nursing care for patients and families in the pre-hospitalization population. This role is ideal for experienced nurses focused on discharge planning and patient education.

• Location: Hartford, CT
• Facility: Hartford Hospital
• Schedule: Full Time, Day shift

• Provide holistic nursing care for patients/families/caregivers prior to hospitalization.
• Complete the Pre-Screening Call and preoperative chart reviews in Epic.
• Identify risk factors and educate patients on discharge planning.
• Assess and identify potential risks and barriers to expected outcomes.
• Partner with a provider to deliver patient care, including phlebotomy and EKG.
• Delegate and supervise work appropriately.
• Function as clinically competent/expert in nursing practice.

• Education: BSN required
• Experience: 3 years’ acute care experience required
