A well-established primary care team is seeking an experienced Family Nurse Practitioner or Physician Assistant to join practices at various locations across Connecticut.
Position Highlights:- 100% outpatient Family Medicine - birth through adult
- Full-time, weekdays only
- No call
- Established, collaborative primary care teams
- Work alongside physicians, APPs, behavioral health providers, pharmacists, and experienced clinical staff
- Strong provider, nursing, MA, and front-office collaboration
- Manage an established patient panel
- Access to multidisciplinary support and specialty resources
- EPIC electronic medical record
