Community Case Manager – Hartford, CT

Delta‑T Group is seeking a Community Case Manager to provide service in the Hartford, CT area.

Full‑schedule client opportunities – Mon – Fri 8:30 – 4:30.

• Identify client needs and establish social services support during housing transition.
• Assist with documentation for securing basic needs and entitlements such as Medicaid and housing subsidies.
• Encourage social supports such as peer activities, groups, and community connections.
• Integrate mental‑health and addiction treatment referrals and wellness services.
• Connect clients with job services and employment alternatives.
• Continue case management activities post‑housing to ensure client success.

• Bachelor's degree strongly preferred.
• At least 1 year of case‑management experience in transitional housing (required).
• Experience working with homeless populations strongly preferred.

• Establish relationship with nation's largest referral agency for behavioral health.
