Parkville Care Center is searching for a full-time Social Worker to facilitate social work services for residents at both Parkville Care Center and Westside Care Center in Hartford, Connecticut. This role entails conducting assessments, planning discharges, and providing direct social services while collaborating with interdisciplinary teams and community resources. A Bachelor's Degree in Social Work is required, along with a minimum of one year’s experience in long-term care.
