Saint Francis Hospital and Medical Center in Hartford, CT is seeking a part-time per diem LCSW/LMSW Social Worker to join our Case Management team. You will assess patients and families, provide direct social work services, and plan discharge alongside the care team.
Responsibilities include ongoing case management, developing discharge plans, communicating with staff and families, using standard social work interventions, and serving as a resource to colleagues throughout hospitalization.
