The Village for Families & Children in Hartford, CT is seeking a Family Navigator to join the Ascend pipeline team. This in-home/community role advocates, mentors, and connects families to local resources, helping them develop and carry out their Plan of Care while building strong community ties.
Responsibilities include recruiting families, coordinating services, and maintaining timely documentation. Bilingual Spanish preferred; a valid driver license and vehicle are required for home visits
