iCare Health Network is seeking a full-time Social Worker in Hartford, CT, preferably Spanish-speaking, to coordinate and supervise Social Work Services for residents in the facility, aligned with state and federal regulations.
Responsibilities include resident interviews and assessments, discharge planning, and collaboration with the interdisciplinary team. A Bachelor’s in Social Work and at least one year in long-term care are required, with Spanish language skills preferred.
