Nurse Navigator

As a Nurse Navigator at Hartford Healthcare, you will guide patients and families through complex care pathways, coordinating services across our hospitals and clinics. You'll provide education, advocacy, and emotional support while ensuring timely appointments, referrals, and follow-up. Working closely with multidisciplinary teams, you'll identify barriers to care, improve care transitions, and promote adherence to treatment plans. Responsibilities

• Serve as primary point of contact guiding patients through the care continuum
• Coordinate appointments, referrals, and follow-up across multidisciplinary teams
• Provide patient education on diagnoses, treatments, and self-care
• Assess patient needs and remove barriers to care, including social determinants
• Collaborate with physicians, nurses, and support staff to optimize care plans
• Monitor patient progress, track outcomes, and escalate concerns promptly
• Document all interactions in the electronic health record accurately and timely
• Participate in quality improvement and care coordination initiatives
