What You’ll Do
  • Evaluate patients and develop individualized physical therapy treatment plans
  • Provide skilled therapy services in patients’ homes
  • Re-evaluate progress and adjust care plans as needed
  • Collaborate with interdisciplinary care team members
  • Educate patients and families on treatment goals and techniques
  • Complete clinical documentation in accordance with agency standards
  • Hourly pay range: $39.25 - $60.84 (based on experience)
  • 6-month housing bonus (conditions apply)
