• Autonomy over your day — structure visits in a way that works for you
• Average visit length of 45 minutes — time to provide thoughtful, patient-centered care
• Daytime visits only — no late evenings or clinic-style shifts
• Flexibility between visits — manage your day without constant pressure
• Strong interdisciplinary team support — independence without being on your own
• Deep patient relationships — care for people in their homes and communities

• Full-time daytime role with flexibility in your schedule
• Consistent patient assignments when possible
• A role that supports work-life balance, not one that competes with it

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide skilled therapy services in patients’ homes
• Re-evaluate progress and adjust care plans as needed
• Collaborate with interdisciplinary care team members
• Educate patients and families on treatment goals and techniques
• Complete clinical documentation in accordance with agency standards
• Hourly pay range: $39.25 - $60.84 (based on experience)
• 6-month housing bonus (conditions apply)
