Therapist – Physical Therapist

Job Summary:

Provides specialized physical therapy services in a home health setting to improve patient mobility and function. Collaborates with an interdisciplinary team to develop and implement individualized treatment plans, ensuring quality care and patient satisfaction.

Responsibilities:

• Evaluate patients’ physical needs and develop personalized treatment plans.
• Administer appropriate therapy interventions following professional standards and agency policies.
• Re-evaluate patient progress regularly and adjust treatment plans accordingly.
• Document clinical assessments, treatment goals, and patient progress accurately.
• Educate patients and their families on treatment procedures and exercises to promote recovery.
• Communicate effectively with healthcare team members to coordinate care.

Qualifications:

• Possess a BS in Physical Therapy or higher degree.
• Hold a valid Physical Therapist license in VT and NH or an unencumbered compact license.
• Demonstrate at least one year of experience in a home healthcare environment.
• Exhibit strong interpersonal and communication skills.
• Show capacity to work independently and as part of a team.
