Dartmouth Health Home Care is seeking a Hospice Social Worker for a full-time day shift. You’ll join a multidisciplinary team and provide clinical social evaluations, advocate for patients, and help coordinate care in the home setting.
Qualifications include an MSW or related master’s degree, at least one year in a health setting, and EMR or database experience. A $10,000 new hire retention bonus applies under certain conditions.
