• Carry out delegated components of occupational therapy evaluations and treatments under supervision.
• Implement treatment plans and therapeutic activities aligned with patient goals.
• Monitor patient progress, adjust interventions as directed, and document outcomes.
• Educate patients and family members on treatment rationale, safety, and home exercise programs.
• Communicate with the interdisciplinary health care team to coordinate care and progress toward discharge.
• Maintain accurate, timely documentation of evaluations, goals, and treatment activities in the patient chart.

• Licensure:
  Must hold an active license as an Occupational Therapist in Vermont and New Hampshire or an unencumbered compact license.
• Education:
  
  BS in Occupational Therapy or postgraduate Occupational Therapy Certification program.
• Certification:
  Must have passed the National Registration Exam.
• Experience:
  
  Home health setting experience is preferred.
• Ability to work in a home healthcare/hospice setting and participate in a travel assignment with day shifts.
