Upper Valley Area
ResponsibilitiesAt the direction of the Clinical Manager, screens, evaluates, re‑evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high quality patient care.

Qualifications- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing
- One year of experience as an RN in a home health care setting is preferred
Licensure/Certification s
- Unencumbered RN license in VT and NH or compact.
- Area of Interest: Nursing
- Pay Range: $85,280.00/Yr. – $/Yr. (Based on 40 hours per week, otherwise pro rata)
- FTE/Hours per pay period: 1.00 – 1.00 – 40 hrs/week
