Hospice Licensed Practical Nurse

Provide compassionate and skilled nursing care to hospice patients in their homes, supporting the interdisciplinary team under the supervision of an RN Case Manager. The LPN will assess patient needs, deliver direct care, and educate families to enhance quality of life during end-of-life stages.

Responsibilities:

  • Conduct home visits to deliver nursing care aligned with established care plans.
  • Communicate patient condition changes and coordinate necessary adjustments with team members and caregivers.
  • Assist the RN or medical provider with specialized duties related to hospice and end-of-life care.
  • Observe, document, and report on patients’ physical and emotional well-being, as well as reactions to care.
  • Educate patients and families on care procedures, symptom management, and comfort strategies.
  • Ensure compliance with all healthcare regulations and maintain detailed, accurate documentation.

Qualifications:

  • Hold an active, unencumbered LPN license in Vermont and New Hampshire or a valid compact license.
  • Graduate from an accredited practical nursing program.
  • Possess experience in medical-surgical nursing and/or home health care environments.
  • Demonstrate strong communication skills and the ability to operate independently within a team.
  • Show sensitivity to patients' emotional and physical needs during end-of-life care.
