Hospice Social Worker - Make a Difference Every Day

Join our team as a Hospice Social Worker and provide compassionate support to patients and their families during life's most challenging moments. You'll guide patients through admissions, care planning, and discharge, provide emotional and psychosocial support, advocate for their rights, and connect them with community resources.

Job Status: Full-time (0.8 FTE, 32 hours per week)

Salary: $27.69 to $34.35/hourly, based on experience

Schedule: M-F, 8AM to 4:30PM

Work Location: Within and surrounding areas of Hartford, Wisconsin

The Hospice Social Worker will assist patients and caregivers/ family in adjusting to and managing changes in patient care needs. This may include, but is not limited to participation in the admission process, collaboration regarding plan of care and discharge planning, while supporting and advocating for the patient's rights.

Essential Job Functions
  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
  • Responsible for on-call services, as assigned.
  • Participate as part of the core team providing cares and services.
  • Participate in the development, review and updating of the Hospice patient's Comprehensive Assessment and the plan of care in accordance with regulatory time frames.
  • Actively participate in coordination of all aspects of patient's care, in accordance with current professional standards and practice.
  • Actively participate in the quality assurance/performance improvement program (QAPI) and manage responsibilities related to Social Services.
  • Provide education and counseling to the patient and, as needed, to the patient's family, consistent with the plan of care.
  • Assist new patients and family with the psychosocial, emotional, spiritual adjustment to changing health and medical needs.
  • Provides assistance to other staff, the patient and his/her support system at the time of admission and prior to the discharge.
  • Participate in education and training of direct care staff regarding Patient Bill of Rights, Advance Directives, and psychosocial needs of the patients and families.
  • Ensure that each patient or responsible party receives a copy of the Patient Bill of Rights, and Advance Directive information.
  • Regularly assess the patient's social service needs; implementing the plan of care provisions to meet and reevaluate the patient's needs and providing ongoing psychosocial assessment of the family's coping capacity relative to the patient's terminal condition.
  • Complete/maintain required documentation in time frame determined by policy.
  • Link patient and family with any needed community resources for ongoing social, emotional and economic needs.
  • Complete or arrange assistance for patient's in completing any necessary forms including advance directives.
  • Compile information on community resources, and keep a current resource file of these services.
  • Maintain honest open professional communication with others including promoting the essence of the interdisciplinary team.
  • Responsible for on-call services, as assigned.
Requirements

Experience and Training
  • Wisconsin Social Worker License (MSW, CSW, APSW, ISW, LCSW).
  • Master's degree preferred.
  • 2-3 years of related work experience preferred.
