Description:
Responsible for providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Utilizes evidence-based practice, clinical decision-making, compassion, and skilled communication to create the safest patient environment and enhance the patient experience. Uses the nursing process to assess the needs of the patient, plan and implement individualized interventions, and evaluate the effectiveness of the plan of care.
Includes the patient and family in planning and implementing care, developing and maintaining a therapeutic relationship. Anticipates and recognizes significant changes in the patient's condition and need for care. Provides ongoing assessment of the patient and takes appropriate actions when changes occur. Alters the plan of care as needed and collaborates with other healthcare team members to coordinate patient-centered care.
Implements strategies to reduce patient risk and increase patient safety while assessing readiness to learn and individualizing the approach. Coordinates the discharge plan utilizing an interdisciplinary approach and applies evidence-based practice to deliver patient care. Incorporates nursing and regulatory standards of care into practice consistently. Conducts ongoing evaluations of patient, team, and unit outcomes and acts when changes are needed.
Participates in shared governance activities and initiatives to improve processes and patient outcomes. Demonstrates effective communication, feedback, and conflict resolution skills. Promotes collaboration among healthcare team members and assumes a leadership role in the care team. Demonstrates a culture of safety by identifying threats to patient safety and intervening to prevent harm. Reports safety events and seeks to implement corrective actions.
Applies ethical decision-making and demonstrates respect for peers and other clinical disciplines. Participates in quality/process improvement initiatives to enhance departmental functions and the patient care experience. Engages in professional activities that contribute to personal development and the development of others. Attends required meetings and completes annual competencies in a timely manner.
