IRIS Program Consultant

Lutheran Social Services seeks an IRIS Program Consultant to support adults with long-term care needs in living independently through Wisconsin's IRIS self-directed program. In this role, you will provide person-centered consulting, assess needs and strengths, and develop individualized service plans that align with participant goals and budgets. You'll coordinate community-based services, educate participants and families on IRIS options, and advocate for choice, dignity, and inclusion.

This hybrid position includes community visits, collaboration with interdisciplinary teams, and detailed documentation. As part of a mission-driven, faith-based nonprofit, you'll help advance social justice while growing professionally in a supportive, values-centered environment.
