Function: The Licensed Practical Nurse (LPN) serves as key support personnel within the Intellectual and Developmental Disabilities (IDD) program. As a vital member of the interdisciplinary team, the LPN provides direct nursing services, ongoing health monitoring, documentation, and staff training, in alignment with Alabama Department of Mental Health (ADMH), Centers for Medicare & Medicaid Services (CMS), and the Alabama Board of Nursing's Nurse Delegation Program (NDP) standards.

Know

How
  • Current and valid LPN license issued by the Alabama Board of Nursing.
  • Successful completion of the ABON Medication Assistance Supervision (MAS) test.
  • Minimum of one (1) year of clinical nursing experience, preferably in IDD or community-based care.
  • Valid Alabama driver's license and insurable driving record.
  • Strong working knowledge of physical assessment, psychotropic medications, and medication administration protocols.
  • Experience with electronic documentation systems (e.g., Therap).
  • Excellent communication, organization, and interpersonal skills.
  • Proficiency in Microsoft Office Suite and documentation platforms.
  • Ability to train and monitor Medication Assistance Certified (MAC) staff and provide delegation per NDP guidelines.
  • Willingness to assume full responsibility for 24-hour on-call duties as required.
Principal Activities
  • Conduct ongoing health assessments
    , including evaluation of vital signs, body systems, and observed health indicators, in accordance with the individual's service plan and regulatory standards.
  • Gather accurate and comprehensive health information to determine the need for referrals, treatment, or follow-up care, ensuring alignment with best practices and person-centered planning.
  • Deliver nursing services to meet the medical and health-related needs of individuals as prescribed by licensed healthcare providers and in compliance with ADMH and CMS standards.
  • Ensure health services and medical records remain compliant with all applicable federal, state, and local regulations, including ADMH, CMS, Medicaid Waiver, licensure, accreditation bodies, and the Alabama Board of Nursing Nurse Delegation Program.
  • Contribute to individualized support planning by providing professional input into the development of health-related outcomes and monitoring strategies.
  • Actively participate in interdisciplinary team (IDT) meetings
    , including individual service plan reviews and behavior support team meetings, offering medical insight and recommendations as needed.
  • Assist in scheduling medical appointments
    , coordinating transportation when necessary, and ensuring timely implementation of physician orders, labs, referrals, and follow-up instructions.
  • Monitor health services delivery through direct observation, documentation review, communication with individuals and staff, and collaboration with medical providers in the community.
  • Provide nursing oversight and follow-up care post-hospitalization, illness, or outpatient procedures, and communicate with the RN regarding any changes in condition or additional nursing needs.
  • Train and educate direct support staff on key medical topics such as medication administration assistance, side effects, infection control protocols, seizure response, lifting, and safe physical assistance techniques.
  • Conduct and document Medication Assistance Certification (MAC) training, delegation, and oversight of certified MAC workers in accordance with the Alabama Nurse Practice Act and Nurse Delegation Program guidelines.
